It is important to reserve your room by the Cut-Off deadline, otherwise a room may no longer be available for you. If you are part of a team, to complete this form, you will need to know the hotel your team has selected.

NO GUEST SHOULD PHONE HOTEL DIRECTLY WITH CREDIT CARD DETAILS. This is done to reduce confusion and, most importantly, avoid double-booking of rooms and any resulting penalties imposed by the hotel.
